Horizon BlueCross BlueShield NJ covers ABA therapy for children with a formal autism spectrum disorder diagnosis under New Jersey's autism insurance mandate. Coverage applies across many Horizon behavioral health plans, including Horizon NJ Direct and managed care network options. Horizon NJ Health, their Medicaid managed care plan, covers ABA for eligible children under 21 through NJ FamilyCare under the EPSDT benefit.
Horizon uses a dedicated ABA Authorization Request Form. This is not a generic prior authorization. Horizon has a specific form for ABA services, and we complete it alongside all required supporting documentation.
Hours are based on medical necessity, not a fixed weekly limit. Your BCBA's clinical recommendation drives the authorized hours.
